How to get an appointment?

Consultation in my office:

·        Please call on 91-731-2400170 between 1000 hrs and 1700 hrs on any working day (Mr Sanjay Chouhan or Mr Praveen Patel) 

·        Please call on 91-9926955065 (Mr Sanjay Chouhan) between 0900 hrs and 2000 hrs ( 9am to 8pm).

Tele-consultation:

·        Please ask one of your relatives/friend in Indore to take an appointment as outlined above. He/She MUST have a copy of your medical records. You should be available on phone at the time of the appointment (Kindly be aware of the time zone differences for the same, India is +5.30 hrs GMT). A prescription will be given to the relative/friend that can be faxed, couriered or scanned and mailed to you.

·        You may send an e-mail directly to me on pravarpassi@gmail.com with “tele-consultation” as the subject. Briefly describe your problem in the e-mail. I will then send you an invoice for the same. Once the payment is received (via www.paypal.com or www.moneybookers.com ), a mutually convenient time can be fixed up. You may need to send me the scanned copies of the relevant papers and reports prior to the telephonic consultation.
